How to view form submissions?

STEPS TO VIEW FORM SUBMISSIONS

Browse Your Practice Online CMS (https://yourpractice.online).
Login using the credentials provided by YPO team.
Click on "Forms" dashboard link to view the list of forms.
Click on "Responses" link to view the form responses.
Click on "View" link to view the detailed response.

Note: Form responses can be exported to excel by clicking on "Export to Excel" link.
